Assist with program planning and implementation. Maintain on-going documentation as indicated - EHR notes, psychosocial updates, individual rights documentation and other needed paperwork. Manage concrete service needs of the individual related to benefits and entitlements. Collaborate with Director of Clinical & Behavioral Services regarding intake and discharge assessment process. Cultivate referral sources for both residential and community day habilitation. Screen potential candidates for goodness of fit with the agency. Gather all necessary documents to be evaluated by the Admissions Committee. Write a report summarizing all the information regarding the potential candidate. Schedule and chair the admission meeting for the community day habilitation. Attend and participate in the Residential Admissions Meetings. Facilitate the integration of new individuals into their residence or program. Work with families regarding the transition process. Ensure that staff adhere to policies and procedures relating to the rights and well-being of individuals. Participate in the Human Rights Committee (HRC) bi-monthly and advocate for least restrictive interventions while maintaining individual safety. Conduct meetings and in-service training including behavior plans, staff orientation, and continuing education, as directed. Provide on-call coverage as assigned by the Director of Clinical & Behavioral Services. Comply with all Opengate's policies and procedures, including Corporate Compliance. Follow federal, state and local regulatory guidelines, pertaining to a safe, healthy and clean work environment. Other duties and tasks as assigned by the Director of Clinical & Behavioral Services. Coordinate the provision of all treatments, activities, experiences, or therapies as prescribed through the clinics admission process. (Article16 Clinic) Coordinate the intake visit on all applicants for clinical services. Ensure the clinical records conform to all agency and regulatory requirements Complete Psychosocial evaluations for clinical needs. Provide therapy, based on the needs of the individuals. Provide supervision to the clinic scheduling coordinator . Psychiatric medication prior authorizations. Work on Psychiatric databases.Willowbrook Class Client Care Manager Applying a person-centered approach Utilizing planning tools such as I AM, CQL POMs, the CAS/DDP2, the LOC, the Comprehensive Emergency Plan and other assessments as needed; such as the Environmental Assessment and Care Giver Adequacy Assessment. Implement, update, and monitor Life Plans, and facilitate individualized Life Plan reviews and approval processes at a minimum of every six months or when a trigger event occurs. Communicate with IDT, physicians, and other providers at regular intervals to monitor and update Life Plan(s) and to advocate for participant needs and preferences. Provide education to participants, caregivers, circles of support, and IDT team. Maintain participant Life Plan and health risk assessment information in secure system and meet all confidentiality requirements. Conduct in-person visits in accordance with OPWDD requirements. Maintain ongoing contact with the important people in a participant’s life, as appropriate. Ensure timely submission of all documentation Assist Individual to ensure maintenance of entitlements including recertifications. guardianship, informed decision making. Maintaining benefits such as Social Security, Supplemental Security Income (SSI), Medicaid and Medicare coverage and/or Food Stamps.
