Very experienced planner with children with disabilities, young people in transition. Excellent reputation for Support Planning across all social care service areas. Lead support planner and the Support Planning Coordinator for Sheffield City Council and lead for Self Directed Support within the Transition Team in Sheffield. Experienced with an excellent knowledge of family carers of children and adults with disabilities. Articles published in publications and books, also produced several media films around social care topics such as carers issues, autism and receiving services. Creator of Support Planning Toolkits and Health Planning Toolkits. A lead implementor of support planning for the personalization agenda for Sheffield City Council. Planning activity covers Children with Disabilities, Autism, Young People in Transition, Care Leavers, Young Offenders and Young People leaving section, people with Mental Health Difficutlies, Older People, People with Phsyical Disabilities and Sensory Impairments as well as people with Health Conditions and End of Life Care. A lead trainer for Sheffield City Council - training both Social Workers/Social Care Workers as well as external Social Care Providers. As well as my professional background I am also the very proud parent of my daughter Clare who has a lifelong disability. Everything I do is always deeply rooted in person centred thinking and a person centred approach with the person central to the decisions made about their lives no matter how much support they need. I believe I have strongly influenced and changed cultures within organisations. I am a driven and passionate person.
