Coordinate workflow to ensure orders are processed and delivered in a timely manner • Resolve staff conflicts and concerns to ensure a safe and appropriate work environment • Evaluate sterile compounding competency for each of the staff member of the department to ensure only competent individuals are making sterile compounds • Handle and address client complaints and concerns. Establish rapport between pharmacy staffs and nursing home staffs. Promote and advocate pharmacy staff members to be valuable team members in providing quality patient care • Oversee quality assurance and implement internal control programs; identify and educate staff members on safety protocols and procedure to minimize dispensing errors • Provide urgent pharmacist coverage for other departments, which processes non-intravenous orders • Responsible for training newly hired pharmacists and serve as a preceptor to pharmacy students. • Perform daily activities of a pharmacist including filling prescriptions, and supervising pharmacy staff while being compliant with the state and federal rules and regulations • Serve as a clinical pharmacist and perform Drug Utilization Review (DUR) for intravenous, oral, and intravenous medication orders and make necessary therapeutic recommendations to providers at Long Term Care Facilities/Nursing Homes/Rehab Centers • Review and evaluate comprehensive metabolic panel (CMP) for patients on Total Parenteral Nutrition (TPN), and recommend new formulations to doctors at long term care facilities. • Provide pain management consultation to prescribers and program Computerized Ambulatory Drug Delivery (CADD) pumps for patient specific orders • Research and answer questions of physicians, nurse practitioners, physician’s assistants and nurses regarding drug therapies, drug interactions, and dose adjustments • Responsible for performing patient-specific compounding services for oral and intravenous medications according to USP standards • Check for completeness and accuracy of all medication fills, including checking content, accuracy of packaging and labeling. • Responsible for performing daily activities of a pharmacist including filling prescriptions, supervising pharmacy staff, and providing customer service • Responsible for administering influenza vaccine • Performing drug-utilization reviews, managing drug interactions, and managing inventory • Counsel patients, answer questions regarding new drug therapies, and make recommendations for over the counter medications • Resolved third party rejections and problems • Train newly hired employees. • Responsible for implementing log copies system • Responsible for answering phone calls and taking verbal prescriptions from the doctor’s office • Assist in training newly hired employees.
